Who Can Benefit From Outpatient Treatment
The Outpatient Clinic at Foundations Behavioral Health offers
expert, confidential care in a private office setting for
children, adolescents and their families in need of emotional
support. This least restrictive level of care can be used
to treat adolescents experiencing emotional, behavioral,
psychiatric, school and family difficulties while living
at home.
Problems that interfere with school and other
successful functioning can be addressed in outpatient
treatment, while
patients maintain their school and family lives. This
treatment offers regular visits with psychiatrists, psychologists,
social workers, counselors and other mental health professionals.
Successful intervention at this level of service may
help
the family avoid other and more restrictive levels of care.
The Treatment Team
Dedicated and skilled professionals work to meet the needs
of each individual to encourage growth, change, and understanding.
The treatment team at Foundations includes the patient,
parent/guardian, psychiatrist, mental health professionals,
educators, and other community representatives. Treatment
is more effective when the patient and parent/guardian
are active participants of the treatment team. Members
of the treatment team are expected to attend all scheduled
appointments, set treatment goals, comply with medication
regimen, and remain compliant with the behavior management
and treatment plan.
Services Offered
The Outpatient Clinic provides psychiatric evaluations, psychological
assessments, medication management, group therapy, individual
psychotherapy, family psychotherapy and nonverbal psychotherapy
(play therapy, art therapy, etc.) Support is provided for
patients whose emotional problems do not require hospitalization.
Therapy is provided by master’s level or doctoral
level therapists.
Services are provided according to the
highest standards of delivery of care and offer the appropriate
support to
the adolescent and his/her family. The staff of the Outpatient
Clinic will work with you to set up appointments that are
convenient and are at an appropriate time to meet your
needs. Patients traditionally attend a one-hour individual
treatment
or group treatment appointment once a week. Following your
initial call, we will also set up a time for your child
to meet with one of our psychiatrists for an evaluation.
Financial Responsibility
Foundations accepts payment from many insurance companies, but the patient’s
parent/guardian may incur financial responsibility for services such as co-pays.
Financial contracts will be arranged to meet these obligations, if necessary.
Foundations offers all services at the lowest possible cost to the patient.
The parent/guardian may be required to pay
a full fee for each session if not covered by insurance.
A fee is charged for any appointment cancelled with less
than 24 hours notice.

Referral Process and Instructions
Why begin the referral process online? There are times when
kids or parents want to start the process of getting help,
but it may be the weekend or evening
when staff members are not always available. Some parents work in settings
where they would rather not share personal information over the phone where
co-workers may overhear. By providing the forms online, we allow you the
opportunity to get us the information necessary to start the process in a
convenient and confidential manner. Sometimes knowing you have begun the
process can help to decrease stress, worry, and anxiety. If you would like
to begin the referral process online, please click the button at the bottom
of the page. You will be asked to read and accept a Privacy Notice. This
Notice is for your protection as you will be asked to submit personal information
via the Internet. We will do our best to ensure that your privacy is maintained
at all times. If you do not feel comfortable with this method, please feel
free to call us at 215-340-1500 x 261 Monday – Friday from 9:00 a.m.-5:00
p.m. Additionally, we want to inform you that we will make every effort to
respond to your inquiry within 2 business days. If you do not hear from our
staff within 2 days of submission, please call us to follow up. After you
accept the Notice, you will be presented with the referral form. Please complete
it fully and follow the directions to submit it. Once submitted, it will
be emailed directly to the Associate Director of Outpatient Services who
will then contact you by phone to make appointment arrangements. You will
also have the availability to download and complete our “Patient and
Family Information” packet. Please print it out and complete it. This
contains information our treatment staff will need at the first appointment.
If your child is in crisis or urgent need
of care, please call your nearest Emergency Room immediately.
